1% Povidone Iodine Mouthwash and Gargle

There has been quite a lot of activity in povidone iodine mouthwash space, and so we thought we’d do a round-up and summary on some of the relevant interviews and studies.

Dr McCullough's interview with Dr Peter Breggin

In this September 14, 2021 interview with Peter Breggin MD, author of Talking Back to Prozac, Dr. Peter McCullough discusses the routine use of diluted (1%) Povidone Iodine mouth wash in the dental office as an excellent preventive measure for COVID-19.

Note: This 10% Povidone Iodine must be diluted by one tenth, to 1% strength before use as a mouth wash, otherwise it is too strong and irritating. Dilute by adding 2 tablespoons (30 ml) of the 10% Povidone Iodine into a full glass of water (250-300 ml). This will make the product diluted enough to use as a mouth rinse, or gargle, (approximately 1.0 to 1.2 %)

2 tablespoons = 30ml povidone iodine
1 glass water = 250-300 ml


Dr. McCullough recommends routine use of this mouthwash on a daily basis, and especially after contact with large indoor groups.

Dr McCullough's interview with Rebel News

In this December 4th, 2021 interview with Rebel News, Peter McCullough MD discusses what is known about the Omicron variant, and the use of Povidone Iodine Mouthwash, Povidone Iodine Nasal Antiseptic Wash and Gargle for prevention of Covid-19. Dr. McCullough cites the Choudhury study in which 600 Covid infected patients were randomized to 1% povidone iodine, or saline placebo mouthwash and gargle. Patients were followed with PCR testing for viral load. Dr. Choudhury writes:

The group of patients used 1% PVP-I [One per cent Povidione Iodine Solution] have shown tremendously reduced mortality, morbidity and hospital as well as financial burden in this covid situation.

Summary of the Choudhury study:

Background: The sudden onset of COVID-19 began in late 2019 caused by a novel coronavirus (SARS-COV2) and on 11th March, WHO declared it to have developed pandemic status. There is still no specific treatment and vaccine available for COVID-19; causing wide spread health problem and concern of the globe. Povidone iodine (PVP-I) is an antiseptic that has been used for over 150 years. It is already proved that different concentration of PVP-I can deactivate COVID-19 virus. 
Methodology: In this randomized controlled clinical trial, out of 1113 patients 606 patients were enrolled and divided in 2 groups by randomization after taken consents. In Gr-A, 303 patients underwent mouthwash/gargle, nasal drops and eye drops with 1% povidone iodine 4 hourly for 4 weeks as well as symptomatic treatment according to need. In Gr-B 303 patients were advised mouthwash/gargle, nasal cavity and eye wash with lukewarm water 4 hourly for 4 weeks and symptomatic treatment according to need. RT-PCR test done every 3rd, 5th and 7th day and Thyroid hormone level (TSH,T3, T4, FT4) at 4th week for follow up. 
Results: The group of patients used 1% PVP-I have shown tremendously reduced mortality, morbidity and hospital as well as financial burden in this covid situation. 
Conclusion: Administration of 1% PVP-I as mouthwash/gargle, nasal or eye drop is simple, rapid and cost effective in reduction of mortality and morbidity by COVID-19.

Another study by Avinash Bidra found complete inactivation of the virus after 15 seconds of gargling with a 1% Povidone Iodine Solution. According to the authors, Povidone Iodine (1%) solution is “rapidly virucidal” at all dilutions tested.



Can povidone iodine mouth-rinse inactivate COVID-19 virus? An evidence-based update

A November 2021 review, published in the Japanese Dental Science Review journal:

The Coronavirus disease in 2019 (COVID-19), also referred to as the novel 'CoV19 (nCov19)' is caused by a new coronavirus strain similar to Severe Acute Respiratory Syndrome (SARS-CoV-2). SARS-CoV-2 spreads via respiratory droplets, saliva, or direct contact. Therefore it is important to control the viral load in the saliva and respiratory secretions. One of the most simple and cost-effective measures that can be adopted by the public and healthcare professionals to prevent cross-contamination and community transmission, is the implementation of effective oral and throat hygiene. Recent evidence has confirmed that 0.5% povidone iodine (PVP-I) mouthrinse/gargle for 30 s can reduce SARS-CoV-2 virus infectivity to below detectable levels. PVP-I can even interrupt SARS-CoV-2 attachment to oral and nasopharyngeal tissues and lower the viral particles in the saliva and respiratory droplets. Thus, the use of PVP-I mouthrinse as a prophylactic measure has been advocated across the globe to reduce disease transmission. Although the efficacy of PVP-I against SARS-CoV-2 is proven, no review articles have yet discussed the evidence and mechanisms of PVP-I against the SARS-CoV-2. Thus, this paper highlights the rationale, safety, recommendations, and dosage of PVP-I gargle/mouthrinse as an effective method to decrease the viral loads during the pressing times of COVID-19.

Conclusion: Oral hygiene intervention, along with the gargling/mouthwash with an antiseptic having virucidal properties is imperative during this pandemic of COVID 19. Based on the Occupational Safety and Health Administration guidelines, PVP-I is safe to use as a mouth rinse, and the existing in-vitro studies and patient-based clinical trials confirm its virucidal property against SARS-CoV-2. In vitro studies have confirmed that 0.5% PVP-I is effective in reducing SARS-CoV-2 in the nasal cavity, nasopharynx, oral cavity, and oropharynx. Since vaccines against SARS-CoV-2 are being rolled and administered globally, the use of PVP-I gargle/mouthwash as a simple, inexpensive, and safe adjunct should be used to reduce the risk of cross-transmission of SARS-CoV-2 viral particles to the community and healthcare professionals. Routine PVP-I gargling/mouthwash/nasal spray can be implemented for all hospitalized and non-hospitalized, symptomatic, and asymptomatic patients infected with SARS-CoV-2 to reduce the viral load. It is equally essential for healthcare professionals (dentists, dental assistants, dental technicians), isolated and quarantined individuals, frequent travellers to use PVP-I mouthrinse to minimize the risk of cross-infection in both the hospital and non-hospital settings. A timely prophylactic oral hygiene regimen using appropriate gargle/mouth rinse, and promoting awareness of the harmful outcomes of poor oral health, may significantly improve the health among vulnerable patients and bridge the gap between oral and general health during the COVID19 pandemic.

My favorite paper is this one from some ENTs in Bangladesh, who diluted common 10%PVP to 0.6% and made it up by the gallon in soda bottles, poured it into plastic sprayers and gave it to hundreds of their patients as a nasal and throat spray. Great pictures. It worked.
